These two are the main/core in my opinion, however there are many more as pointed out by others like MQM, guns, drugs, mishandling of refugees, bombings (I believe that phenomena started in his era too), not to forget flaming of sectarian violence etc. Need more?

okay I'll start with trivial/baby-step: It was wrong to indoctrinate whole nation towards the "jihad" and start recruiting common people and let anyone start operating unsupervised/uncontrolled training centres.
