She probably is giving the MCAT to get entrance to Pakistani medical schools, they have their own version of MCAT. You give it after intermediate, which is when you are about 18 years old, so she probably isn’t thinking of going to USA now. that said, getting married to an Indian when you are a Pakistani living in Pakistan is hard. I am guessing this guy wants to go to Dubai etc at some point that’s why he is talking of living outside India. Remember, for both sides, it’s very hard to go visit the other side. For example if you want to go see your parents, your husband will have a hard time getting his visa and same for you if you want to visit India.
I would suggest, just wait, this is merely infatuation and probably will fizzle out with time. If it does not and you still want to marry him after you are done with med school, then think about it.
