I don't think It's possible to send a picture from your PC to telenor, mobilink, ufone etc via the Internet. Just send a MMS from your own mobile.

How are you keying the PAK number? Write It here as you key, maybe you are not Including the International dialling code.

for international mobile numbers you should always use +92321. you can get the "+" sign by pressing and holding "0" or in some sets by pressing "0" a number of times

call your service provider
ask them if they have global partnetrtship with telenore for mms service.

Sending MMS outside the country is usually tricky & doesnt work many times. Check if MMS is available on Telenor, if MMS is not available then you might not be able to.
If you are based in North America then forget sending MMS to anywhere but North America. North American countries lack in GSM techologies and Mobile network infrastructure is still 5 years behind than Europe/Asia.
