A husband doesn't HAVE to tell his wife he got a 2nd one, however it is expected out of good manners and respect.
Also, the permission of the first wife is not required. There is no evidence to show this in quran or hadith, it is a choice that the man makes. Also, even if the first wife doesn't like it, the husband can still get another one since he does not have to obey his wife. Furthermore, the wife doesn't have a valid right to disagree to the husbands marriage as this is something that Allah made halal for men.
HOWEVER............. If the husband and wife put this in their marriage contract, that he will not take a 2nd wife, then he is obliged to honor his contract. Otherwise, it's fair game.

Most men who will marry a second wife dont even feel they need to tell the first wife, as [some believe] first wife's permission is not required/needed, and usually good manners and respect go out the window.
The reasons for polygamy were to help widows and orphans, who did not have the means to be financially independent to support themselves or their famillies.
Also, marrying another woman because your first wife cannot concieve just seems selfish to me personally mainly due to the millions and millions of orphans out there. Its far better to adopt a child, give them a home, love, a future, and raise them to be good Muslims rather than they end up becoming drug addicts or prostitutes or whatever.
Marrying another woman because you're too horny isn't really allowed, although some idiots m ight tell you otherwise.

You still haven't answered what YOU would do; Assume that he comes to you and says "Sara jaanu, I want to marry this other girl. Are you okay with it?" Again, keeping in mind he will treat everyone equally.

No where in the quran or hadith does it say your second wife must be a widow. Having children is everyone's right. It's one of the mercy's from Allah onto his slaves. Who are you to say some should give that up.

Everyone has the ability to differentiate between right and wrong.** If hte first wife [who is barren] objects to the second marriage, I think she has every right to object and make her feelings known on the matter. And if her husband STILL goes ahead and marries knowing that he has disrespected her and not treated her the way she deserves then that is wrong.

The only purpose is to instigate and we don't have a right to say we don't like a ruling that Allah made, like oh i like alcohol, it sucks that it's haram.

In reality, how many women do you know who've adopted sons will be doing purdah in front of him, even if she wears hijab? How common and normal do you think it'll be that an old man will be marrying a girl he raised, his own daughter, if not in teh biological sense but in every other sense fo the word? Kabhi akal ki baat hoti hai.
