Just for information ,

  1. IVF is even successful when sperm count is very lower means lowest ?

2.Whast the average cost and any effects and after effects on couple ?

Im not sure that IVF is 100%. As in the first time is done it will be successful! maybe someone else can shed some light on that!
The cost for the IVF is $4k each time.

After that the couple will have to pay for the IVF which may prove difficult as you can imagine!

Stonecold, IVF chances are 1 in 3 of it being successful. That doesn't mean that it will be successful on the 3rd cycle, it could be on the 1st, or 2nd or 15th or none at all.

I would advise the couple to take the NHS IVF option as soon as possible, as it will probably be a waiting list, while still working on the the current sperm meds. So in case the 1st round of IVF doesn't work, in that time the meds should have increased the count anyway, so no need for IVF?

Also in that time, couple can slowly save up for a next IVF cycle if necessary.

Personally I am of the belief that when Allah wills that he will bless a couple with a baby, it will happen and nothing can stop it, so keep your hopes and prayers up, inshallah it will happen when the time is right.

The second test of 3mil showed more than just "an average". The seems to have been a 5% increase.

As you can imagine 15 times of IVF can be trying on any couple!

Theres no reason to stop taking the medicine yeah? It helped, and hopefully will continue to help. Maybe seek a new Urologist too, a second opinion never hurts and a different doc may be more helpful in figuring out why the problem is there and possible other solutions. Meantime, it may also be a good idea to get on the waiting list for IVF. Why not, right?

Agree with above said comments. The couple should probably take the medicine for a few months and then sign up for ivf..(which gives them few more months and a better chance) for conceiving!
