Such incidents are actually results of the wrong steps which were taken earlier.
With no offense to women, but I think I should mention this here as I also have mentioned this problem to my mother, sister and also to my wife regarding incident 1. Why do some woman have to keep a very low neck and then cover it with dupatta only to keep correcting dupatta again and again very conciously in all public places. IT sounds similar to a boy who is walking around wearing a pant with opened zip and trying to cover it with his sweater. WHY on earth can't he just close the zip and make things easier for himself.
Though I do understand that at times it is the mistake of the tailer, may be tailers have made such mistakes very frequently.
