You submit a Form Bay, along with 2 copies of your passport pictures (please no niqab), and a written request to close your account, one copy in Urdu, and another in English (certified, and notarized). Your forms will be reviewed and will be put in a queue. Right now, GS is very backed up, and is processing July 1998 applicants.
