I don't know about Dr Shazia. But I took an epidural in both deliveries ( not in Pakistan). After that you won't feel the internal exams or the water breaking procedure. Even in Pakistan in larger hospitals at least they try to do non emergency c- sections with epidurals. At least in Karachi, epidurals for childbirth have been around for almost 20 years.
But it is said that they won't give you epidural until you are 5cm dilated, and what before that? Most of the times they break your water right in the beginning. Getting epidural too early will slow down the process and will lead to c-section.

The contractions get worse as you progress in labor. It used to be in the U.S. They used to give epidural after 4 cm but now whenever you want one you get it . As they now believe c sections don't rise because of early epidurals as previously believed. I got my water broken further into labor and with my second one ( after the epidural) my water broke on its own. At least you will get an epidural even if it's after 5 cm. don't go to the hospital too early unless otherwise directed or there is bleeding or your water breaks. Go when the contractions are five minutes apart. That way hopefully you might be further along when you get to the hospital.
