I agree with first post that Pakistan win will be dependent on Umpire fairness, or in other words if equal number of contentious decision go in favor of both teams, and secondly on Pakistan catches and run outs.
As far as team changes are concern, I think Pakistan should bring in FARHAT, HAFEEZ, RAZZAQ and AKHTAR for BUTT, UMAR, IQBAL and SAMI. If AKHTAR is not ready then Pakisan should stay with same 4 bowlers as from last test. Otherwise logic for these changes is:
Farhat seems in better touch with bat compare to Butt, Hafeez better bowler and fielder than Umar, Inclusion of Razzaq will strengthen batting along with an additional support to bowlings if Akhtar break down mid way. And Akhtar inclusion could be a surprise weapon.
Though I wish if Pakisan could try Niazi.

LAHORE: Pacers Rana Naveed-ul-Hassan and Muhammad Asif left tonight for England to take part in The Oval Test, to be played from August 17.
Last week, Asif had cleared PCB medical commission’s fitness test, but team management requested the board not to send him for the upcoming Test. However, the management changed its stance and again requested the board to book the pacer along with Rana Naveed for the final Test.
After getting the fitness certificate, the two pacers said, they were completely fit and ready to play for the team.

^ I think they are misinformed sources, they will likely participate in ODI series..... but there is always space for our think-empty-tank's mind games suggesting they will not play last Test but suddenly they'll appear in playing XI.

If Shoaib Akhtar has completely recovered from his injury, then he should play. The only question is his match preparedness due to him being out of action for 6 months. Frankly, I'd take a half-prepared Shoaib than a fully fit Sami any day.
Even if Shoaib plays, PCB isn't likely to drop Sami to make room for him. The axe would fall on poor Shahid Nazir.
Including Razzaq neither strengthens the bowling, nor the batting. Test cricket is a specialist's game, though an argument can be made for Afridi since he can turn a match on its head in an hour or two.

Totally AGREE with smooth_guy, Kaneria should be rested and this case team look good on paper. But I fear ENG will go for dead or fast pitch. Now they have no fear of loosing the series. Smooth Guy on paper your proposed team will be best suited for dead or fast pitch. On fast Pak will have luxary of 4 seamers, and on dead pitch they will have 6 work horses.

Shoiab Actor out of the final test. There is uncertainty about the other superstars Asif and Rana. This means there is every possibility that Mr Sami will play.
Hafeez will be opening with Farhat. Another new combination. Butt, Niazi and Taufeeq sent home.
