first i think its Pakistanis own fault that let the outsider get involve in east Pakistan by providing them an open oppertunity by not accepting Mujeeb Govt.
Indians were actively seeking revenge from the 65 war. Pakistan intellegence should have interuppted such RAW missions to create cells who supported all this.
3rdly one cant fight with own people. a political issue and ground reality wasnt accepted by West Pakistani leadership and its all Bhuttos ego and yahya khan in ability.
with so much vested interst in east Pakistan, India successfully created, do all believe that it was only Pakistani Army done the crimes against east Pakistanis?? and how easy for India to flare things up more by carry out such attacks to create more enmity against countrymen from east to west, and blame that on Pak Army to create more voilance,..
definitely there must have been some voilations by few Army men too, linked to all war like situations.

Failure to solve issues politically once esclated.
Failure to keep indians out of it and defending East Pakistan.
