Did you change it from the 'My Profile'?**
There are two things you can check.
One in user setttings make sure that the checkbox to display avatar is checked.
Second, in the settings of avatar itself (where you change avatar), the option to display default avatar is not chosen. HTH
if you have uploaded custom then make sure image size is exactly 50 by 50.
and name does not contains space and/or - in the name (to be on the safe side)
