With your permission Muslim 786, I wish to make a few corrections to what you stated:
There are 5 fundamentals of Muslim faith:
(1) to beieve in God
(2) to believe in His Angels
(3) to believe in His Holy Books
(4) to believe in His messengers
(5) to believe in the day of judgement
Practically, A Muslim is liable:
(1) to declare his faith in oneness of God and the prophethood of Mohammad (PBUH). "Shahada(h)"
(2) to perform "Sala(t)" i.e. to lead one's life as per Islamic law (Persian word "Nimaz" is only for the ritual which is only a small part part of "Sala(t)".
(3) to practice "Saum" in the month of Ramadan
(4) to perform "Hujj" (at least once in a life time).
(5) to pay the governmental taxes "Zaka(t)"
Jihad and sacrifice are the aspects which surrounds the whole human life every-time, everywhere! but talking about the fundamentals one must take care as these are mentioned in Quran in this particular order.
