I’m ashamed to say I can barely read/write urdu, can anyone tell me what it says to fill out on the NICOP form in urdu?
Re: What does it say in urdu on the ID card form?
Here is NICOP form (both in English & Urdu). You only need to fill in ONE language, either English or Urdu. No need to worry about what it says in Urdu, if you are filling it in English.

Section 'd' of the instruction says Both the English and Urdu parts are compulsory. The US version has been given a lil makeover, the UK one is of less quality.

But my mum told me what they mean: my name, mum's names, dad's name, spouse's name.

yes your mom is right....you have to answer those questions both in english and urdu...meaning once u have finished writing ur name etc in english...you will then have to write it in urdu aswell in the space provided.

Re: What does it say in urdu on the ID card form?
If you live at place where you have access to NADRA swift registration centre (London has one) than you do not need to fill any form. Just visit the centre with documents and all will be dealt there. You do not even have to take your photograph as that would be taken by them. You will need to go there once. NICOP would be sent to you at your home.

Re: What does it say in urdu on the ID card form?
you still must require a non blood relative attester? I think the office is central, I don't want to go there, and have to go there again cause of some complication, I think I'll miss the computerised system (how stupid I done computer science)

Have a trip. You will require no attestation. Take all required documents and that is all. Make sure you reach NADRA swift registration centre before 12 AM and I believe all would be over by 3 PM (at most). Then wait for couple of weeks and ID would reach you at home.

You are welcome. If you are in UK, here is website of Pakistan high commission from where you can get relevant information. Any problem, you can ring them, especially NADRA department, and ask for details. I found them very cooperative, so I believe you would get similar cooperation from them.

If you are in other country then find about them from Pakistan embassy. What I know, they are present in most countries Pakistan has relation.
