If u take shower than u have to do wadu, but if u took Ghusal then u dont have to do wudu.
Now ur next question might be wats ghusal?? well ghusal is also shower but u have to follow some steps and directions thats it, then u dont need wudu.

can you shower with the intention of praying right after - instead of taking an extra step to do wadu - although you are as clean as you can be

If between your shower and the time you pray, if you haven't done anything that nullifies wudhu then you can pray in that condition. Of course this is assuming that during your shower, you wet all your parts that are required to be wet in wudhu.
If u have done wudu n glanced at some nude stuff unconsciously, then what u need to do -- a revision of wudu.

None of that actually requires a fresh wudhu. I don't know where this type of thinking comes from. This one brother I knew thought that even hearing a girl's name would require him to make a fresh wudhu. Then another friend would say girls' names to him before the time of salat and he would run off to make wudhu.

Scholars even say that the if you walk out in a rain, or accidentally fall into pool of clean water, and your full face, all four limbs (feet till ankle and hands till elbows) and hair got wet, you can perform salat and dont need a fresh wudu. Apparently to do wudu, you dont need a prior niyat

Perhaps it depends on the madhab. I believe in Shafi madhab, having the correct intention is a rukn, and also doing the washings in the correct order is a rukn, so then falling into a puddle does not seem to suffice
