That is a big question. It is interesting, you run away at times after asking question and do not provide details.
Get him seen by a urologist.
Many reasons for it and different treatments.
A one sided varicocele is at times more serious.

I am sure the doctor who diagnosed him must have told him a solution as well !
Doctor told him to use tight underwear.
Or a support device against gravity, yes. But he needs more thorough check up if not already done. The problem may be higher in the stomach area. If not treated or improved he may develop bad testes. Good luck for your friend.

That is just to make a diagnosis. Left sided are much more common and are not always serious. I am not saying something might be there but need to be evaluated by a good **urologist **if not done already. If the person is older than 40, that makes it more serious.
