Urinary Tract Infection

Hi,

I have severe back pain because of UTI, my doctor prescribed me antibiotics for 10 days but I was still in pain and use Tylenol to reduce pain, after 10 days doctor said u need more antibiotics for 10 days but I am still in pain, taking medicine is not enough to reduce pain, the pain is unbearable, need help.

Uti rarely cause persistent backache. First urine urine RE is required to rule in/out an infection or a stone. Now pain due to urinary system happens in particular areas called renal angles. Pain due to UTI is rather too acute (it's called acute Pyelonephritis) and is associated with other signs of infection ie swinging fever, nausea or vomiting and abdominal pain too - in absence of later symptoms unlikely it's UTI causing backache. Stone is then a possibility requires a CT -KUB (kidney ureter bladder) for diagnosis and stone removal either laser guided or thru surgery depending upon type of stone.
But Urinary tract pain is usually one sided - if both sides pain is present then the cause is unlikely kidneys etc - maybe musculoskeletal backache or pain from thorax.
Nonetheless backache usually requires NSAids like Naprosyn ( don't use it if you are asthmatic or have precious stomach ulcers etc) you can chat with your doctor along above lines and see what differential diagnosis he's got for you, it's certainly doesn't seem like a UTI.

Thanks for detailed reply, yeah she did KUB, no stone X-Ray is clear, just UTI, starts from 4th of May still in pain after taking antibiotics, use ing Tylenol to remove pain, life is miserable now a days. What can I do to stop pain ? Final diagnosis is anemia and UTI, no stone but pain is causing disturbance. After every 6-7 hrs I took pain killers.

Pain is on right side not backache and sometimes right front, some time in muscles and sometimes in bone area.

Sorry to hear that. It's really strange that you had a month of UTI, is there any chance that you can do a urine culture and sensitivity to know exactly why persistent UTI symptoms. There's a condition called bladder spasm syndrome in which you get all signs and symptoms of UTI for weeks but no laboratory evidence of UTI.
So to control pain it's essential to treat the cause. Secondly you can add an NSAID like Naproxen 500 mg twice daily after food (in Pakistan the trade name for Naproxen that I know of is Naprosyn) it hopefully will be useful - again in assuming your pain is indeed urinary tract in origin.

Thanks for reply. Actually my doctor checked again the sample for uti, and she said u have to take another course of antibiotic so first I did ciprofloxacin twice a day for 10 days now levofloxacin once daily for 10 days today is the third day of taking levo. I am tired of taking pain killer, tomorrow I will talk to the nurse, hopefully in few day I will be better, In Shaa Allah.

Ahan, I may come across as trying to discredit your doctor but I am not doing that. But in all honesty he's not doing a good job (you suffering from a UTI for a month is evidence enough for that!).
ciprofloxacin and Levofloxacin are both quinolones category of antibiotics (note similar names) and have exactly same spectrum (ie kill same sort of bacteria- usually what are called Gram Negative Bacteria) if cipro wasn't able to do the job Levofloxacin will not be able to do that too. So usually when one type of antibiotics fail then we try a different type and usually a broad spectrum one(ie activity against more Gram Negative bugs but also Gram Positive bugs). One example is Augmentin (625mg three times a day for 5 -7days).
Usually when we send urine for lab analysis - lab identifies what bug is it - and then we can target that bug with antibiotics that the bug is sensitive to. That's why I asked if urine culture is done - culture is different from Urine RE which is very basic.

You need a CT-KUB, USG won't help much.

How did it go? Given duration of symptoms - Hopefully CT KUB- which I suggested very early actually - will also comment on other structures ie spine genital tract and gastro-intestinal system in its field of view - as after this prolonged episode - I doubt Nephrolithiasis (kidney stone) alone would have done it. Hope you get some answers.

Tomorrow I will go for CT Scan procedure.
