If India, then go to any of their stores ..(Though I think he's not doing this style anarkali anymore and I am not sure if he'll do made to order.)

USA: PiakaGhar or TheGrandtrunk..(Latter being the cheaper). Piakaghar charges almost double of what it retails for in stores, even though she has a ton of his stuff.
Google both and you should be able to get more Info on Location..etc
