For the most part it is not a symptom of an underlying medical condition…at least I am not aware of any.
It is usually either due to hereditary, lack of sleep, or allergies.

if under eye puffiness the only thing present than it's nothing serious but if it comes with a baggage of other symptoms and signs of fluid retention in body mostly due to HTN , thyroid or kidney disease than better consult a doctor for that.
Water retention too.. have it checked out, if it isn't a med condition place u're regular eye-gel in the fridge and use it every night!

with other symptoms i meant if a person has any other underlying disease which could lead to under eye puffiness includes;
- thyroid disorder…hypothyroidism Hypothyroidism - Wikipedia
- kidney disease… nephrotic syndrome Nephrotic syndrome - Wikipedia
- edema in body Edema - Wikipedia
- certain medications also have this side effect ( if u’ve this concern than better go thru the handout that comes with yr prescribed medication , it should be mentioned there )
