Mine MOST embarrassing moment was in 2005 at the Karachi guppy meet. I can’t mention it as it involves a guppan 
Other than that there are probably many but some of those are when I walked into ladies restroom not realizing that i was entering the ladies restroom. No lady ever saw me going in or coming out (without using of course) but this one time I walked into a bathroom (in a hospital’s emergency room - for both genders) after knocking the door. Well there was no response, then I tried to open the door and it opened as it was unlocked, when I stepped in, there was a woman DOING MAKE UP! I was so embarrassed but she said that’s okay. I am about to leave, you can come in
