What is the general feeling about returning to a firm from which you were downsized a few years back?

I wouldn't. Simply because I don't want to work in an environment where I wouldn't feel comfortable. And I would find it hard to feel comfortable working for an ex-employer.

Well....most of the folks that were there when I was, are now gone. According to the new VP of Sales, the company has done a 180 degree turn in it's approach.

I've only had one meeting yet and I'm still confused as to what I should do.

You mean all the people who made you leave are now gone themselves? In that case I'd do some "independent" research whether the approach of the company has really changed that dramatically.
