I always tell, cz I keep zakat monies separate from normal sadqa and khairaat. Kinda makes sense to me. And if someone doesn't want to accept zakat, I'd rather respect that as well.

It is not neccessary to tell the payee that it is zakat money, but it is better to tell.
More important is ke you should know where you can give zakat.
For example you can not give zakat or sadqa to your masjid’s fund, which will be used for running expenses. Also you can’t give to non-muslims (although sadqa can be given to them)
The best disposal of zakat is to give it to your needy relatives, needy neighbors. If there is no one needy among them, then other muslims
