Yeah it happens but still very difficult unless at a very malignant program. And usually theyll give you the option to voluntarily resign because that allows you to find another residency program as opposed to getting kicked out which would be a huge red flag. Usually it’s a trend in professional issues (showing up late to work, not showing up at all, not answering pages, showing attitude to attending, not putting in medical orders, etc). Even substance abuse (i.e. showing up drunk to hospital or taking a medication from hospital and self injecting) won’t get you fired at most programs as long it didn’t directly impact patient care–you’ll have to take time off at least 1 year to get help, will get referred to the states medical board and go through rehab even if its a 1st time offense and prove that you have learned from your mistakes and are suitable to practice before you get reinstated.
