& what do you do about the hurt and more importantly, how do you prevent that individual from doing that again to you or to someone else?

Learn your lesson and move on if it was due to your own negligence. If another individual was the cause, warn them. If they dont listen, leave it to God. He will balance things out for you sooner or later.

it depends upon how important other person is to me, i used to get disturb with even a tiny thing if someone ever said against me , but i've learnt a lot with time and know that i can't shut up everyone's mouth , so if the other person isn't close enough or sincere to me than whatever his/ her foul mouth say i don't pay heed ( or if possible try to make him/ her learn a lesson , in a worst way i can ), on the other hand if the person is closer to me , whether a friend or anyone from family , and get bad feeling about me or due to some misunderstanding my reputation get ruined b/c of that , that i'd definitely speak up to him/ her to sort out the differeances and do damage control if any has happened to my reputation ....
the thing is people who r close to me know me well so i know my reputation would hardly be effected in front of them,.................. baqi bharr main jain

perhaps, the best thing is to always first put the personal responsibility before all else, regardless of how others are behaving or acting.

and how people’s perceptions are often times wrong about us or about themselves?
where do these factors fit in when personal reputation is the issue at hand?
a very good argument someone makes is this: 'i dont care what others think, since only i know, what i did/ do or will do and with what intentions'.
should not people who perceive or judge or misjudge others, be mindful of this particular reason that people do what they do, because they might have a valid reason for doing things?
if people did care for someone they claim they do, then they wont leave a person isolated or negatively view that person.
instead they would step up and try their best to understand the person.
and help her or him feel supported, even with just as much as not being viewed negatively or prejudged.

Personal reputation. Personally, I hold it very dear to my heart. However, it is not the reputation which others have about me that is close to my heart. My personal reputation in front of my own judging self is the all important reputation to me.
I TRY to understand and improve myself when I find someone hitting on my reputation. I always find in such cases part of the fault definitely emanating from my own self.
