The interesting thing about routes to Pakistan is, that generally airlines require reciprocity. So, if PIA is serving Karachi-Melbourne (lets say), Qantas will want reciprocity to serve Karachi from Melbourne too. However, most airlines shy away from serving Pakistan. Thus PIA gets a monopoly over the route. Karachi-New York is a perfect example. PIA is the only airline that flies that route. No American airline even comes to Pakistan.
