Hello there,

my cousin has lost his B-form (NADRA). Now we need it, cause of some visa issues. Can we get a new one? Help! :frowning:

Form B is for kids under 12, required for their passport. Is this the same one?http://www.parep.org.sg/form/passportAppB.pdf

or is it the one for the birth of a child? http://www.parep.org.sg/form/childReg.pdf

Silaaj, my cousin has to submit a few certificates and forms for a visa in a schengen country. One of the requirments are B-form (of father). Ab the B-form is lost, what to do? :expressionless:

Visit NADRA Baji and they will tell you. Duplicate should not be an issue and cost is also not very high.

If you guys are not in Pakistan, parents or adult siblings should be able to get the copy.

Yeah we're not in pakistan, but someone just told us a B-form duplicate cannot be given... O.o Also, the NADRA people never pick up their phone!!!! Argh!!!

they never answer their phones. try sending some one to NADRA or check with the embassy in your country.

No personal experience but dont think that duplicate should be an issue.
