**YES, to change name in the passport from the one u've currently on your birth certificate. you also need to take with you the 'marriage certificate' in order to facilitate the change.
since i'm NOT in the UK ***** i don't know where to get the name change but in Canada it's done at the region's office of Birth Registration in the Ministry of Citizenship and Naturalization. u may look up in the 'Blue" Govenmental pages of your fone book or look up on the internet. u'll find it, iA. ***
