I have a question about Dhuhar and Asr Prayers. Suppose If I joined Dhuhar or Asr prayers which was already started and I realized I missed first rakat. Do I read Fatiha + a Surah at last after salam? I mean in first two rakat we read Fatiha + Surah and in last two just Fatiha. What is the correct way to read/offer it.

Yes you have to read fatiha + surah.
say we have 1-2-3-4 Rikaats, when you miss 1 you have to complete 1 after the 4. I hope it is clear.

What if I joined the salat and don't know how many rakat were offerend and how many are left? Lets say I joined the the salah in third rakat but while joining I wasn't aware of number of rakat so I read Fatiha+surah. In that situation what will be the correct way?
Actually I'm trying to view it from all possible angle.

^ Why do you recite when following imam? I follow ahnaf and thus we keep quite when in jammaat, if you do not follow a madhab/maslak, i will search for other opinions for you inshallah.

Asalaamu Alaikum, when you are praying behind the imaam you do not need to recite anything as the Imaams recitation counts for all those praying behind him.
If you are praying a 4 raka'ah Fard prayer behind an imaam and you missed one raka'ah then you simply get up after the Imaam has done Salaam and then pray the first raka'ah that you missed = Surah fatiha +surah.
If you missed 2 raka'ah's then you get up after Imaam has done Salaam and pray the first two raka'ah's that you missed = Surah Fatiha + Surah's.
If you missed 3 raka'ah's then you get up after the Imaam has done Salaam and pray the third raka'ah first so you only recite surah faatiha in that raka'ah and then stay down and recite "Athahiyathoo" and then get up after and pray the first 2 raka'ahs as normal = Surah fatiha + surah.
If you missed 4 raka'ahs then after the Imaam has done Salaam you get up and pray all 4 rakahs as normal.
