As far as I know, ta in dtap is for tetanus..it's diphtheria, tetanus, and pertussis... Pertussis is the whooping cough, that you should have the vaccine for your baby.
Meningitis... I think you can buy (menactra) here in the us but not confirmed.
Nada..please, for your girls sake, find a good doc in Pakistan.. Seriously they don't live in caves back home.

Assuming she will be 4 months old when you leave for Pakistan...most likely she will have received rota vaccine.Depending on the type,it can be 2 or 3 doses. My daughter got 2 doses and was done by 4 months of age (in the USA).Ask your pediatrician if she needs 3 doses or 2.
She will be young for flu shot (given after 6 months of age)and way too young for typhoid vaccine (given after 2 years of age). She will also be young for Hepatitis A vaccine.
As Sahar said there is no vaccine she needs before traveling to Pakistan, assuming we are talking about the 4 month old baby.
Regarding Hepatitis B,she will be able to get the first 2 doses before she leaves UK.
Peshawar is one of the main cities of Pakistan. It has medical colleges and hospitals. So for sure it has some doctors too.To date all med schools in Pakistan have English as the medium of instruction. So students going there graduate and do know how to speak English.I very well recall you mentioning once that you were unable to find doctors who spoke other than Pushto.I hope you get to find one and get satisfactory consultations.
Please do not feel offended, but my parent's have a driver who comes from Mardan.Even a smaller city like Mardan is able to offer 'hifazatee teekay'.I am not sure which ones ,but I assume they all follow the same WHO recommendations,I also believe there might be reputable places in Peshawar. You just might have to do some extensive homework. All the best...:)
