Intikhab Alam was only offered the job as Pakistan coach after Javed Miandad turned down an offer of a fourth stint in the position. Ijaz Butt, the PCB chairman, said Miandad had been Pakistan’s first choice to replace Geoff Lawson, who was sacked as the coach on Friday.
“Intikhab was our second choice after we first went for Javd Miandad,” Butt told Bigstarcricket.com. “But it was very good of Javed to tell us that he was not interested in the position and that he would rather help us in other ways in an advisory capacity.
“Intikhab will first take charge of the team in Abu Dhabi but I hope that he will remain in the post for two years but this needs to be cleared by the board first. I know that Intikhab has an excellent rapport with the players and I am keen for him to take Pakistan cricket back to where it belongs, and to give us the success we should be having.”
Miandad most recently held the coaching role from 2003 to 2004, when he was eventually replaced by Bob Woolmer. He had also been coach in 1998-99 and 2000-01. Intikhab has also had a previous spell as the team’s coach, from 1999 to 2000.

Well, I am glad to see that there were "choices", first-choice/second/third, in Ejaz Butt's personal opinion. I mean, it wouldn't have made sense to have gone through a review process anyways because Butt had it figured out.

Recently Saleem Altaf was made the Director-General of PCB, newly named COO of the company (CricInfo). Javed Miandad has now replaced Saleem Altaf for the position of Director-General. Javed Miandad, the former Pakistan captain, has been named the Pakistan board’s director-general, replacing Saleem Altaf, who had recently been named in the same role, but will now take charge as chief operating officer. Confusingly, Altaf’s new position had originally been scrapped when his appointment as director-general was made.
The rejigging proved necessary because Miandad had been highly tipped to take over as Pakistan’s coach following the dismissal of Geoff Lawson last month, but to the PCB’s disappointment, he turned down a fourth stint in charge of the team, instead offering to help the board in an advisory position. (CricInfo)
Is Saleem Altaf no longer working for PCB? Has PCB broken down COO and Director-General into two separate positions or is Javed appointed for both, Director-General and COO?
Javed Miandad, director-general of the PCB, has proposed to strengthen Pakistan’s domestic cricket by encouraging the game at the grass-roots level. Among Miandad’s goals are to modify the existing domestic cricket format to expose cricketers from various regions and revive the standard of bank and business-sponsored teams (CricInfo).
Recently appointed cricket ambassador to China, Miandad has promised his full support in promoting the game in China. “I have prepared a plan for the development of the game and I will start it very soon.”
