sometimes, taKhallus was used in the matl’a of the Ghazal. one such she’r is from AsGhar GonDvii:
** Khuda jaane kahaaN hai “AsGhar”-e-Deevaana barsoN se
**
**k jisko DhooNDhte haiN Ka’ba-o-butKhaana barsoN se
**

yes in rare cases… there are more than one ashaar in a ghazal containing name of poet.. Ahmed Faraz’s famous ghazal ‘Zindagi se yehi gila hai mujhe’ got two maqtaas:

Koh kan ho ke, Qais ho ke Faraz
sab main ik shakhs hi mila hai mujhe

second one is:

KON JANE KE CHAHTON MEIN FARAZ
KIYA GANWAYA HAI KIYA MILA HAI MUJHE

There is Pakistani poet Adeem Hashmi (whose ghazal Kat hi gai Judai bhi is popular). He wrote a ghazal and each shair contain his name.
