I am planning to visit Lahore next month. Kindly tell me the followings:
1 Which Hotels are good to stay. Give me some names of 4 Star and 5 Star hotels in Lahore and give some recommendation.
2 Which restaurants are good. Where to break fast where to lunch and where to dinner.
3 Is breakfast free in 4 or 5 Star Hotels or no.
4 Good places to visit in Lahore.
5 Good places for shopping.

So I am assuming you don’t have family in Lahore that you can stay with. Which hotel you should go for depends on your budget. The best and therefore also the most expensive hotels are:
However there is a new chain of budget hotels called “Hotel One” that has been launched by the Pearl Continental Group, which is supposed to be quite nice. It’s not uber luxury, but being associated with the Pearl Continental Group they still maintain a decent level of services and cleanliness. My cousin has stayed at one of those and found them very good.

In terms of restaurants: I like the ones on MM Alam Road. The food is relatively safe and should not cause your stomach any issues. But for the “true” Lahori taste you would have to go Downtown. And there you need to have a strong stomach. 
Whether breakfast is free or not depends on the hotel. So once you decide which one you wanna go for, check with them.
Places to visit in Lahore are too many to name them all here. There was a thread a couple of years ago where a few of those places were listed. Here it is.

The Village restaurant in lahore is a worthwhile buffet style restaurant. cuisine was desi from what I recall.

1.Avari and PC situated at the Mall Road is the best hotels of Lahore!
2. Depends on which kind of food you like The best one is in Fortress opposite Bundu Khan MM Alam road has many restaurants please visit there

3: You might want to check with Gourmet or Rahat, if they have cakes for diabetics too. If they don't you will probably not find them anywhere in Lahore.

Yes, Actually it is Turkish Dish but it is very famous in all the Arab countries.

There is a couple of Lebanese restaurants in Lahore. The ones I know are on MM Alam Road and are pretty good, but also expensive (for Lahori standards).

Like every one is mentioning here, the hotels in mm alam road including village, zayafat, Bundu khan (moon market and fortress stadium), salt and pepper near liberty market are good. Hotels include avari, pc, best western, holiday inn, ambassador etc. as far as shawarma is concerned cock and bull is the best, you can find it in Gulberg as well as Faisal town.

Hotel Sun fort in Liberty is also reasonable. I’d prefer to live near liberty, or Mall road. Most probably Liberty, as thats the center of Lahore and you can find places to eat and shop close by. As far as shopping is concerned; Liberty, Gulberg main Boulevard, Mall Road, Faisal Town Link road, Karim Block Market and Barkat market etc.
