You can do it yourself. It's just a 2 rakat namaz. Someone correct me if I'm wrong, but I've read that in the first rakat you recite surah Kafiroon after surah Fatiha. And then in the second rakat, you recite surah Ikhlas after surah Fatiha. After praying, you recite the dua.
its just 2 rakat namaz (nafal) .. .. preferable offered after isha .. and there is not restriction on surat etc. Offer your namaz, then in dua, offer the particular dua for istikhara and then sleep ..
