depends on the nature of the relationship. If the person avoding you is otherwise very close to you, definitely ask. If you hardly know the person who you think is avoiding you, simply ingore him. Who cares.

Depends on your relation, and the strength of urge you feel about asking. Its better to clear up the things, so that any kind of misunderstanding may not take place.
