It depends upon the offense and expectation if it can be forgiven multiple times or not.
If my wife expects me to call her once every hour and I keep forgetting that, I dont think it should be a big deal cause the expectation is not realistic.
On the other hand, if she finds me cheating, once could be forgiven, two times should be more than enough and then she can be the judge of whatever she wants to do
