It will be applied to Malaysian Immigration Authority on your behalf by your Malaysian employer if you have applied the job directly. They will send necessary paperwork so you go to Malaysian High Commission an get it stamped. OR in the other case it will be processed by authorized local agent in Pakistan (called overseas employment promoter). The Pakistani agent must be registered (licensed) by Ministry of Overseas Pakistanis and FIA. DO NOT hand over money to anyone. Malaysian Govt has strict regulations for overseas recruiting. It is done through appointed agents who then give quotas to Pakistani promoters.
You will also have to get PROTECTORATE stamp from Pakistani Immigration. If you don't have this stamp in passport, Pakistan embassy won't assist you in any trouble while in Malaysia. It is a must for people going for work.
