They are available in both languages.
Urdu, so at least that's the one i've seen from pakistan. I'm pretty sure it's in urdu.

Both the languages. Its best to fill out both the forms, signature on both... pata nahi kab kon sa kaam aa jaey. Don't have to worry about going into translations and stuff
btw guys this is a question out of this topic But are the nikkah namas in Pakistan - in English or urdu?

Each company or masjid has their own legal papers and stamps etc and u can ask them for an english only document or a document with both urdu and english.. or in urdu only.
