Anything that causes pain & injury to self & others in wilfull act of S&M is wrong. If I burn myself with a cigarette or choke another to the point of his passing out is neither a rush nor a pleasure. It is self mutilation, self-flagellation and can lead to murder.

free of any constraint or pressure, overt or covert?
the only thing that should matter, or should there also be other consideraions that regulate mutual consent?

Let's break up BDS&M into its two respective parts and discuss separately.
For those who may be unfamiliar with the lingo of S&M - it means any sado-masochistic acts that derives pleasures from hurting others or self in the process.
There can be no CONSENT on activities that inflicts physical or mental torture & pain upon another or one-self. Examples like - Drawing blood till one passes out; throwing dagger at one tied to a roulette wheel; or forcefully holding one's breath to the edge of death, all done to experience that 'rush' or 'on the edge of death experience' cannot be considered normal acts. If you want to catch that 'rush' then, get into sports.
Bondage & Dominance or B&D is mostly role-playing practised by sexual partners where one partner ties up the other or cuffs the other; whip him/her with soft leather; or have the partner lock her/him up as sex-slave in a cage, etc are role-playing. However, hurt your partner and you will be prosecuted by the law even if you are two consenting adult.
S&M is of the very extreme variety and that is why I would call those eager to participate as being of unsound mind needing help.

BD & SM are two seprate things. You seem to be mixing up BD with S&M. Some one, like a lawyer or professional going to a dominatrix is not same as engaging in S&M or ritual mutilation.
B&D involves role-playing and sure pleasure is exchanged for a sum of money; A dominatrix whips one with soft leather whips and dresses up in leathers and talks tough & dirty; the trip does not give one physical scars or mental scars. Do a little research.

There can NEVER be mutual consent to these kinds of behaviour - look it up. Just like there can never be consent for suicides & murders.
People who visit dominatrix or play dominatrix with their partners are usually in it for the fetish or the sexual thrills that comes from a domineering or subservient sexual role (B&D). Here it is a matter of consenting adults but even then in cases of injuries charges are enforceable.

Actually, that is a good point! Many do indeed consider Boxing as a pugilistic & sado-masochistic sport and would like to see it banned. Sports are not played with intention to cause pain or mutilation and are supervised with thousand in attendence.
It would be a mistake to link general competitive sports to S&M. The biggest diference is - S&M is about inflicting pain upon the other or to one-self as the title of this thread goes - "I want to do terrible things to you".
If you can wilfully hurt or cause pain to yourself or one in your group then you are fully capable of hurting, maiming others in general public and find pleasure from it. This can never be termed a civilized persuit aimed at seeking pleasure and mutual consent for harmful activities are never legally consideted consent.
No, pain is pain, whether 'monitored' by thousands of spectators or none at all. It's inflicted upon an individual for the purpose of amusement, whether it's sports or something more personal as sexual intercourse.
You are only unable to see the similarities because you are used to see and accept boxing (say) but not any other form of infliction of pain. A boxer knows very well it's gonna hurt once he/she gets into the ring but he/she willingly does so - in this case, boxers derive amusement out of competitiveness.
