How were the names of Surahs of Quaran determined?

Who decided upon names of these Surahs? When?

As I understand, some Surahs of Quran were revealed in chunks over a time period. How were they compiled into one chapter?

Prophet Muhammad SAW used to tell companions that this ayah belongs to such and such surah. Evidence is the Surah Baqarah, which revealed over 9 years in Madinah. There were several other Surah that were revealed in parallel during that time. The names of the surahs are mostly aliases due to their unique subject.
