No...I got a Samsung phone in Feb, (start of 2 year contract), then I upgraded to Galaxy Note in Sept (re-started a 2 year contract) and asked them for the unlock code so I could give the phone to a friend, who is on Verizon. That is when I learned that no unlock code is necessary for Samsungs, unless the earlier phone is to be used on ATT, with a different phone number. I am not under contract for 2 phones, just the one. Maybe they are cool with it because I continue to be under contract for at least one phone. I don't know.
I thought that it was only the iphoen that uses a SIM in verizon, though that SIM is just for storing some personal phone data (4G LTE through sim?) and is not a GSM SIM unless your in europe. Basically, you can't convert a GSM phone to a CDMA.
CDMA and GSM have been in iphone since version 4. It is the firmware that has to be unlocked even if it is unlocked. I don’t think that you can just take a verizon phone to at&t now can you?

CDMA and GSM since iphone 4S, and yes you can use verizon iphone 5 to att and use the sim card. data speed will be less i think its 4G instead of LTE.
iPhone 5 on Verizon has an open (unlocked) GSM slot. You can put in AT&T, Tmobile or any other SIM from around the world (cut into the right size ofcourse) into that phone and use it.
I don't know what it would do if you kept both Verizon and ATT lines active and if they would work concurrently or not.

GSM service will always take presence over CDMA. as soon as you put it a sim it will turn the CDMA off and Try to run with GSM.
