Pakistanis who know English and are educated in Pakistan, but come to live in UK, USA or Canada or any other English speaking country. How hard do they find it and do they understand local dialects? My wife can speak, write and understand English, but when watching a movie in a Texas accent I think she has no clue what there saying but pretends to know.

I love the Texas accent (and how nice are the people!!) but I think it could be hard for a non-native English speaker to follow sometimes just as some of our UK accents can be difficult to follow even for those of us born and bred here..

How about a Glaswegian film? Even Scots in Edinburgh find it hard to understand.
