I happen to attend a summit on Anti Corruption in eastern part of US last month; one of the issues discussed in three day summit was strong correlation between the following which resulted in failed states; in which African countries were quite prominent and only two were from Southern Asia (Afghanistan & Pakistan):
Unaccountable (Power Base and Governing)
Façade of Legitimacy (E.g., Democracy, Rule of Law etc.,) for optics
Are established countries leading the world failing to help the failing states or are they failing because of their own reasons in general; who has more onus for failures?

The list is not in order of importance and also "unaccountable" was used in a different context where public office holders are so aloof that no system can touch them. Hope this helps!
There are multiple factors but only few are "illustrious enough" of failing - again and again!
But if you want to draw parallels between Pakistan and USA Governments- it's your pov and prerogative and you are entitled to it.

All this discussion about failed states is a fad. I dont think a single Pakistani would consider Pakistan to be a failed state. True there are problems but the situation is not out of control. I think it is more of a fear mongering.
It is my POV and hopefully I am also entitled to it.

This failed nation nonsense is just that. Pakistan's growing. Failed states arent building infrastructure, dont have functioning govt, dont have any institutions. To lump Pakistan in with Afghanistan is beyond stupid.
