No, you only need to apply for it once every two years. I had applied once in May 2009 but I have travelled to the USA many times since.

It's valid for two years. Doesn't matter if you're travelling with a different airline. :)

I think ESTA is not for pakistan....... I dunno all conturies but its UK and more countries, u can look on their website.

ESTA only works for citizens of countries that are part of the US Visa Waiver program. Obviously Pakistan is not one of those countries and therefore ESTA won't work for Pakistani citizens.
For those who have already submitted their ESTA request in the past: It remains valid for 2 years. It doesn't matter how often and on how many different airlines you travel. It just has to be valid. If it's expired, you need to apply for a new ESTA.
In the past the ESTA application used to be free of charge. However the Department of Homeland Security has recently introduced a US$ 14 charge for an ESTA application. That's a one-time payment and after that the ESTA remains valid for 2 years, just like it was previously.

The US$ 14 fee was introduced on September 8th 2010. However all ESTA forms that have been authorized prior to that date remain valid.

Ideally one should update ESTA information ie flight and address details. that will make sense, but even if you dont, your Airline will definitely will be doing that automatically by providing your and your ESTA details to US authorities before your departure to US.
