Inspi, report him for sure! There is no excuse, no justification for this kind of behavior in the workplace. And you are not even pally-pally with this guy, that he can say such things as a (bad) joke.
The onus of caring about his career is on him, not you. If he's a physiotherapist he's definitely educated enough to understand that such language can lead him into trouble.
I have a very relaxed team at work and we joke A LOT…race, religion, sex…nothing’s really off limits. But even with what I’m around daily…I’m telling you what that guy did to you crosses the line big time! There is nothing that justifies what he did. If he honestly thought it was some type of “joke”…then that’s even more reason for him to have a “talk” with HR!
I just doubted myself for a bit because its his career, and I think he does think its some sort of 'joke/chatupline', because our working environment is pretty chilled too.

I've been through something like this at work and I didn't report it the first time. Because of that, there was a second time and it started to extend to other colleagues of mine.
It was the same issue, he was a managing director and I felt bad about his career being over and what it might do in the future for him. But the thing is...if he isn't worried about it (these people are well aware of what the consequences are for such actions) then you shouldn't be either.
I reported him and after I did...35 other people followed suit. He was fired that same week.
^OMG. What a creep! Was he passing sexual remarks and that sort? And whoa, 35 other people?! Good for all of you for standing up and doing the right thing. The nerve of some people. =/
I'd inform the authorities but would emphasize more on the action coming as a severe last warning than anything else. Losing a job over something that could well be attributed to a single disturbing/disgusting slip of tongue incident could get pretty ugly for a guy in his position/post.
