I want to know if the prophet (pbuh) was foretold of his actual death ? I know he had prepared his last sermon as if he was about to die, but was there an actual revelation to him ?
I am also aware that he died pretty young. Was he sick when he died ?

Many hadees indicate that Prophet :saw: knew his time to meet his lord is near.

As per hadith(close meaning to it) : he(saw) was given a choice to live forever in this world or to go meet Allah(swt). He chose to go to Allah(swt).
On another note : When the angel of death descended on prophets, they took their permission before taking their souls, but for regular people like you and I, the angel of death will come unannounced and for us ther will be no options.
Mohammed(saw) died because he(saw) was poisoned by a jewish woman.

i seriously doubt this fairytale.. men are mortals.. all prophets were men and were mortal.. never did God send us a prophet who was otherwise.
As the Qur’an explains, The Prophet was instructed to tell us who he was:
[17:93] Or you should have a house of gold, or you should ascend into heaven, and we will not believe in your ascending until you bring down to us a book which we may read. Say: Glory be to my Lord; am I aught but a mortal messenger?
This urge to make Kings and Gods out of a messenger, the desire to see him perform miracles, or other supernatural acts, or to make him out to be a ‘superhuman’ is a trait of the disbelievers.. they refused to believe the message just because it had come to them through an ordinary person .. a mortal just like them..
[17:94] And nothing prevented people from believing when the guidance came to them except that they said: What! has Allah raised up a mortal to be a messenger?
Apparently after the prophets death, the disbelievers managed to infiltrate the Muslim belief with these tales.. sadly many muslims today embrace them as their religion and throw the Qur’an aside.

Prophets (AS) do have Miracles or ‘Mujzat’. No question about it. and its not difficult to understand nor there any complications.

I quoted from the seerah of Mohammed (saw) by Hijaazi and by Imam Anwar Al Awlaki. It is in the hadith. Like you, I sincerely and firmly believe that all prophets were mortals.
