You can go to the NADRA office to get it done. It’s quicker that way, since their is less chance of mistake on the computer form.
If you are overseas, you can get this form.
^ u misunderstood my query bro'. i have **nicop* and poc. i want to make computerised shanakhti card, i was hoping to find a application form for that but i couldn't.*

You have to go to the local NADRA office or post office to get the form in person.

Either you can download the Form from the website of NADRA or get these from the Embassy. Your mailign address is not known otherwise I would have posted you.
fOR getting your NIOP you have to fillup two Forms. One is for NICOP and other for NIC. In case you have already got your new computerised NIC then you do not need to submit this Form). If you have not yet got your Computerised NIC in Pakistan then you will have to submit your above application/form in Original. This Form cannot be photocopied. Form for NICOP can be used photocopied.
You will submit your Forms to your Embassy. It is the safest and best way otherwise you can send it directly toNADRA Islamabad goetherwith a Fee of US$15/-.
NADRA will make two IDs for you. One NICOP which will come back to you through the Embassy. The second (computerised NIC) they would keep with them and on your permanent return to Pakistan will take your NIOP and give your Computerised NIC.
