I saw them doing that on the food network once in some competition. There they poured cream in such a way that it created designs. The designs weren't as complex though, they did flowers, hearts something that looked like an onion sliced lengthwise.

I saw the stencils for the coffee decoration here in a Japanese store, they looked cute.

Its done with a very thin stick like pencil
Straight after the froth is poured, use the pencil to make the design making

sometimes they pour caramel over the froth and make the design with that but most of the time they use the coffee for the outlines and leave the froth to fill in..

but you can make heart shape without the stick by pouring the froth in a way that it creates a heart shape.
