as far as kids’ [2-12] fare is concerned, it’s 75% what whatever the adult fare is. babies [up to 2 years] pay 10% of the adult fare that you are paying.

as a general rule: the fares to gateways in a country are always cheaper. gateways are different for different airlines. since most western and US airlines do NOT fly to Pakistan, you have a limited choice and range in fares.
Karachi will definitely be less expensive. flying into ISB, depending on the airline you use, could a nominally cheaper. ONLY Middle Eastern airlines fly to Lahore as i understand.
your best bet would be Middle Eastern Airlines like Etihad, Emirates or Qatar Airways.
flying out of LAX will be the cheapest and inbound journey, if taken after January 9, will cost you much lower.
