The following example uses the APA format for the journal citation. NOTE: APA requires double spacing within citations.
Waite, L. J., Goldschneider, F. K., & Witsberger, C. (1986). Nonfamily living and the erosion of traditional family orientations among young adults. American Sociological Review, 51, 541-554.
The authors, researchers at the Rand Corporation and Brown University, use data from the National Longitudinal Surveys of Young Women and Young Men to test their hypothesis that nonfamily living by young adults alters their attitudes, values, plans, and expectations, moving them away from their belief in traditional sex roles. They find their hypothesis strongly supported in young females, while the effects were fewer in studies of young males. Increasing the time away from parents before marrying increased individualism, self-sufficiency, and changes in attitudes about families. In contrast, an earlier study by Williams cited below shows no significant gender differences in sex role attitudes as a result of nonfamily living.

Yep.. as we know , a bibliography is a list of sources (books, journals, websites, periodicals, etc.) we use for researching a topic. Often called as "references" or "works cited" .. where an annotation is a summary and/or evaluation.
Therefore, an annotated bibliography includes a summary and/or evaluation of each of the sources.

what I reckon .. you should summarize your source.
Argue and discuss main arguments and topics your source cover… Same as you are introducing your source to some xyz fella …. Try to evaluate it.
Critically analysis it , if Is it a useful source or not . .. later on describe how it fits into your research. Was this source helpful to you and How does it help you shape your argument…..

it requires proper referencing (APA, MLA or chicago style). and then you have to talk about how those scholarly resources gonna be used in your work.
intro to writer is very handy.
AB is not more than 5-6 pages if your resources are 10. (so for five references, 2-3 pages i guess).
i use MLA, and my sis uses APA. it depends on the course linkage. cuz if it is history-humanity kinda then i think u have to use MLA. . you have to consult your teacher-prof for this 
so AB is a way to find out execution of research in your paper
