What is the best way to approach for a raise. Should I talk verbally or rather create a folder illustrating all the work I have done so far. I really don’t know what is the pressional way of doing this.

You need to build a case to talk about salary. Normally, you would discuss it at your annual review with your boss..u should however hint him/her that you would like to discuss your accomplishments at some point.
Before you meet your boss, you should know what your expectations should be. Market rate, experience, number of years invested in the company, skills acquired and accomplishments (hard & soft) all add up.

^ agree with Fayz on the items that need to be incorporated when preparing your case for a raise.
In terms of approaching your superiors, it really depends on your relationship with him/her. I’ve been in situations where I’ve approached them informally first - and that usually leads to a more formal meeting in due course where both of us get a chance to do our homeworks and discuss our views.
The annual review is always a good opportunity to present your portfolio.

I am sort of stuck in a rut right now. My manager that I had been reporting for past 2.5 years left for another job about 5 months ago. Now we have a new manager who has been here only 1.5 months and does not understand whole a lot and does not know me too well. Before my previous manager left she gave me raise around April and she told me I will get another raise again and another one at end of the year.
Now she is gone and God’s knows what happened to that promise ![]()
Now I have to prove myself again to my new manager and all that hard work to get a raise. Since my manager left, scope of my work has completely changed and what I am doing definitely does not fit the profile of this salary. I have been working on these projects for 6 months now. But since my new manager has been here for 1.5 months only there is so much I can make him understand that I deserve a raise again. I don’t know where to begin to talk to him. Or should I present all the projects i have been working on past 6 months so he can look at them.

Your manager must be going through an adjustment phase himself. You should definitely prepare what you have done in the last 6 months and wait for the right time to bring it up. Timing of discussion and his understanding of your company's environment are critical for him to understand your achievements.
I would also look for whats out there in the market...no harm in interviewing for better financial and growth opportunities.

There is no right time or wrong time to ask for the raise. It's all about documenting your case and presenting it at an appropriate time.
Remember your manger is not going to read through pages and pages of documents. You should create a powerpoint deck or a short document identifying your commitment, your belief in the team/company and your achievments. You need to present yourself as someone going above and beyond and deserving that raise that meets the market value. Be sure to attach all the supporting documents, such as project plans, emails, etc. as evidence of your superb work.
Another thing you could do is ask your colleagues to rate your performance. The people you have collaborated with in your company and outside can help you get what your want. You don't need to tell them why you are seeking the answers, just in tell them that you trying to improve yourself. When seeking such review, be narrow and direct, example "It was great working with you on Project X. During this project, I learned a lot from you and I would like to improve myself with your help, can you please answer the following questions;
Q1) how did I perform? (Poor, Good, Exceptional)
Q2) How are my team skills? (Poor, Good, Exceptional)
Q3) Was I a critial member of the team? Yes/No
Any Comments: ...........................
If some give you good comment, you can use them in your document as an evidence. If the review is not so good, just file it far away :)
