We traveled to China via bus the very first year when the crossing was officially opened for tourists. It was back in the summer of 86' I believe. The road was fine until Khunjerab, but totally sucked afterwards into China. Actually, some sections between Khunjerab and Tashgurkan had no road at all. The bus would stop and the bull-dozers cleared the boulders in front. From Tashgurkan to Kashi, the road was OK.

The old Chinese bus broke down in the middle of no-where somewhere close to the Tibetan plateau OR it ran out of diesel; can't really remember. Nonetheless, we made it in one piece to Kashghar, and stayed there for about 2 weeks. The whole ordeal was one of the most thrilling yet exhausting trips of my life; the scenery; the desolation of those areas was simply breath-taking.

In order to fend off altitude sickness, make sure to take lots of dried apricots on your way from Hunza or Gilgit. It will save you from excruciating headaches when you are above 12,000 ft level.
